Lumenos HSA Plans are a health savings account (HSA) plan that provides generous, comprehensive benefits after the deductible is satisfied. Anthem Lumenos Plans are consumer-driven health plans that give you more control and information to help maintain and improve your health. The plans utilize Health Savings Accounts (HSAs) to help control your health care costs while still providing great, comprehensive coverage. This gives you the opportunity to put tax-deductible money aside to pay medical expenses, while also saving money on your health insurance.
There are two Lumenos HSA Plans to choose from - Lumenos HSA 100% Plan and the Lumenos HSA 70% Plan.
These plans include:
- 100% coverage for recommended preventive care services (including annual physicals) when you see an in-network provider (deductible waived)
- Traditional Health Coverage to help cover additional health care expenses
- A single customer service organization for both the health insurance and the health account components of Lumenos plans

The following is a list of covered preventive care services for adults included in all Lumenos Plans:
Office Visits (for preventive physical exams
Screening Tests including:
- Eye chart vision screening (Full vision exams not included)
- Hearing screening
- Cholesterol and Lipid level screening
- Blood Glucose test to screen for Type II Diabetes
- Prostate Cancer screenings including Digital Rectal Exam and PSA test
- Breast exam and Mammography screening
- Pelvic exam, Pap test and contraceptive management for females
- Screening for sexually transmitted diseases
- HIV test
- Bone Density test to screen for osteoporosis
- Colorectal Cancer screening including Fecal Occult Blood test, Barium Enema, Flexible Sigmoidoscopy and screening Colonoscopy
- Routine Blood and urine screenings
Immunizations for:
- Hepatitis A
- Hepatitis B
- Tetanus, Diphtheria (Td)
- Varicella (chicken pox)
- Influenza (flu shot)
- Pneumococcal Conjugate (pneumonia)
- Humana Papilloma Virus (HPV)
- Measles, Mumps, Rubella (MMR)
- Meningococcal Polysaccharide
- Herpes Zoster (shingles)
Child Preventive Care
The following is a list of covered preventive care services for babies and children included in all Lumenos Plans:
Office Visits (for preventive physical exams
Screening Tests including:
- Eye chart vision screening (Full vision exams not included)

- Pelvic exam, Pap test and contraceptive management for females
Immunizations for:
- Hepatitis A
- Hepatitis B
- Diphtheria, Tetanus, Pertussis (DtaP)
- Varicella (chicken pox)
- Influenza (flu shot)
- Pneumococcal Conjugate (pneumonia)
- H.Influenza type b
- Polio
- Measles, Mumps, Rubella (MMR)
- Meningococcal Polysaccharide
- Rotavirus
HSA Administration
Setting up a Health Savings Account (HSA) is optional, but it is highly recommended in order to utilize the tax advantages of this plan. If you want to open an HSA, you have the option of electing Anthem's integrated HSA from Mellon Bank or any bank or financial institution of your choice. However, there are some advantages of electing Anthem's integrated HSA from Mellon Bank, which they will help set up your account during the application process.
